Throughout this material, you will find a brief synopsis of programs designed to benefit Dobson employees. From health insurance to retirement and tuition reimbursement, Dobson Communications has made an investment in its most valuable resource - its people.

Dobson offers a generous selection of employment benefits to eligible employees, their spouse and dependents, including all of the following:

Group Health Insurance
The Company offers health, dental, vision and prescription coverage for full-time and part time (30+ hours) employees. The health plan features a PPO network through Blue Cross Blue Shield. Group health benefits require an employee contribution each pay period. Employee coverage is $15 per pay period, employee plus coverage for one dependent is $30 per pay period, and family coverage is $45 per pay period. Employees enrolling spouses on the plan may have an additional surcharge if the spouse has waived other available group health coverage to have primary coverage through Dobson's plan. Contributions for group health benefits are taken on a pre-tax basis.

Group Life Insurance
The Company pays 100% of premiums for employee group life coverage for full-time and part time (30+ hours) employees. Benefits are four times your annual salary - plus bonuses and commissions - up to $350,000. Also, Accidental Death and Dismemberment coverage benefits are four times your annual earnings up to a maximum of $350,000. The current plan features a PPO Network with a choice of providers/specialists within the network. The plan also contains Life Insurance Benefit (burial) in the amount of $10,000 for spouse, $5,000 per child over six months of age and $1,000 for a child seven days to six months. Employees are covered after the waiting period.

Short/Long-Term Disability Insurance
The Company pays 100% of premiums for short and long term disability insurance coverage. For those employees who have at least one year of service, STD coverage provides for 60% replacement of lost earnings due to a disability after a 14 day waiting period has elapsed. Employees are eligible for long-term disability coverage after a 180 day waiting period.

Flexible Spending Account
The Company pays 100% of administration fees for this benefit, which provides employees with the opportunity to participate in a compensation alternative plan under the IRS 125 Cafeteria Plan. This allows for the utilization of pre-tax income to cover certain medical and dependent care expenses not covered by other benefit plans.

401K Retirement Plan
The Company pays 100% of all administration fees for this plan. An employee is eligible to make pre-tax contributions and receive a Company matching contribution at the semi-annual plan entry date following the end of the first full year of employment. The company will match up to 4% of all contributions made to this plan dollar for dollar. The employer-matching portion is vested at 100% upon enrollment.

Employee Assistance Program
The Company pays 100% of the administration fees for an Employee Assistance Program providing confidential assessments and counseling for full and part-time employees and immediate family members. The Company pays for the first three assessment visits, if required. Additional sessions may be covered under the Company's group health plan.

Paid Time Off
Paid Time Off (PTO) provides all full and part-time (30+ hours) employees with paid time away from work. This paid time offers employees the flexibility to schedule their time to use towards a personal vacation, personal illness, assisting a family member with their needs, PTA meetings, appointments, etc. The Company provides PTO accruals based on years of service.

Tuition Reimbursement Program
Eligible employees who qualify for the Tuition Reimbursement Program are assisted with tuition costs and related fees for job or career-related degree programs at an accredited institution. The Program covers up to $2,500 per calendar year, plus up to $25 per course towards books. Employees are limited to two courses per semester.
